  • Pharmacy processing fees some insurers don't cover on first fills

Refills skip the supplies. Refills also sometimes qualify for manufacturer discounts or coupon programs that don't apply to initial fills.

Insurance Covers Less at First

Many insurance plans treat first fills as non-covered or partially-covered. The policy terms often exclude starter kits and initial setup costs. Subsequent fills fall under standard prescription coverage once you're established on the medication.

What You Can Do

Ask your pharmacy or telehealth provider upfront about total out-of-pocket cost including supplies. Check whether the manufacturer offers first-fill discounts (Novo Nordisk, Eli Lilly and other makers run these periodically). Request generic alternatives if your insurance covers them. Compare prices across pharmacies before filling, especially for mail order options that sometimes undercut retail.

Know your deductible status. If you haven't met your annual deductible, your first fill may cost more because you're paying the full negotiated rate until deductible clears.

Second and third fills typically cost less.
